Pre and Post Tests

Objectives

The objectives of our programs are to teach students the importance of making healthy choices throughout their everyday life. Students travel through the entire process from beginning to end with our awesome High-Flying Kid Motivators. Students will be armed with the knowledge to incorporate healthy decision-making skills in their everyday life.

Instructions

Complete the post-test evaluation 2 weeks after your field trip. Distribute each survey enclosed to each student who has attended the selected HealthWorks! program.

When delivering the survey please inform the students of the following:
1) The test will not count as part of the child’s final class grade
2) Students should place an identifier on top of each page. This will be their school name and grade (ex. GreenPark2nd).

While administering the survey each teacher should read the questions out loud to the students. Once the surveys are complete teachers should place them back in the envelope and seal it. The front of envelopes should have the school name and number of surveys completed. Each envelope is postmarked ready for delivery back to HealthWorks! Please deposit the envelope in the outgoing mail once the surveys are completed.
